Disease Management
The TriSurant definition of Disease Management is a program which
proactively identifies patients with certain chronic conditions
including:
- Diabetes
- Asthma
- Congestive Heart Failure (CHF)
- Chronic Obstructive Pulmonary Disease (COPD)
- Coronary Artery Disease (CAD)
- Depression
- Hypertension (High Blood Pressure)
and provides early and consistent interventions to make a significant
difference in the health of the person while reducing the costs
related to such conditions.
TriSurant has collaborated with a vendor partner to offer our
clients a URAC Accredited Disease Management program. This URAC
certified program uses a predictive modeling tool to identify
the members with these chronic conditions and stratify their risk
level to identify the appropriate level of intervention for each
member. Management of the member’s condition happens through
member education, counseling and continued assessment to improve
their health status. Results are evident in improved clinical
outcomes and member feedback. These actions help to avoid costly
emergency room visits and inpatient admissions, and they work
to lower overall claims costs. With an increased understanding
of their condition, an ongoing individualized program, and improved
compliancy of their medical regimen, each member can achieve improved
outcomes and reach the goal of maintaining maximum health.
Diabetes is a significant health issue in our country. In addition
to the diabetes support programs available in the information
center and our disease management program, TriSurant provides
a comprehensive and integrated program for the care and control
of diabetes. The program identifies, manages, measures, and improves
patient outcomes. Medical, educational, and nutritional interventions
are targeted to the prevention of diabetes and to improvements
in diabetes health and wellness. On-site retinal, foot, and cardiology
care complement comprehensive medical, educational, nutritional,
exercise and lifestyle programs.
